Serving the Blue Water area for over 25 years, Mercy Home Care and Mercy Hospice is committed to our community. We servie St. Clair, Sanilac and Macomb counties, as well as parts of Lapeer county.

A member of Trinity Home Health Services and affiliated with St. Joseph Mercy Port Huron, the Mercy Home Care team consists of nurses, physical therapists, occupational therapists, speech and language pathologists, social workers and home health aides.  Mercy Hospice, provides nurses, aides, social workers, bereavement counselors, and volunteers to assist patients with life-limiting illnesses and their families. Our staff lives where our patients live, and we have a RN available 24/7 to assist you!

Mercy Home Care and Mercy Hospice are organized as not-for-profit, faith-based organizations.

The Mercy Home Care Partners in Care Program is a unique program designed to serve patients with chronic or terminal diagnosis with extended pain management and social/spiritual support. Mercy Home Care also offers a number of specialized programs including Hearts at Home, Beyond Balance, Low Vision, Pulmonary Rehab, Incontinence, and Diabetes programs. These programs are designed to help patients with activities of daily living, understanding their diagnosis, coping with new challenges and reducing hospital readmissions.  The Mercy Home Care team also has specially trained wound care certified nurses to assist patients with complex wounds or those that are not healing quickly. All of Mercy Home Care’s RNs are trained annually on infusion techniques, wound VAC technologies, and ostomy care. The Mercy Home Care Total Joint Care Program allows patients recovering from a total joint procedure to have experienced physical and occupational therapists help with the complete care from blood draws, staple/suture removal and rehab services.

Mercy Hospice staff will work with our patient’s physician or our medical directors to provide compassionate care in a location of the patient’s choice. While many patients choose to stay at their home, we often have patients who would prefer to reside at a skilled nursing facility or an assisted living facility. Mercy Hospice can work with patients in any of these locations. Our care goes beyond care of the patient, it includes bereavement for the family and support during the illness.
